1. Print out the following:

• Staff Application
• Statement of Faith
• 3 Reference Recommendation Forms

2. Fill out the Staff Application and include the following:

• A recent photo of yourself
• Your signature on the Statement of Faith release form

3. Mail all forms to:

Children of the Nations
Attn: Human Resources (or the name of the COTN staff you’ve been in contact with)
P.O. Box 3970
Silverdale, WA 98383

4. Give Reference Recommendation forms to 3 references. Ask references to complete form and mail it to Children of the Nations.
