• Release Form Packet

2. Give the reference forms to three references. Ask each of them to complete the form and mail it directly to Children of the Nations.

3. Send the following completed forms to Children of the Nations at the address listed below:

• Team Leader Application
• Release Form Signature Page
• $50 application fee
• $200 non-refundable deposit

4. Approved applicants submit the following to COTN:

• Medical Release Form (doctor’s signature required)
• Two copies of your passport
